Please read this contract carefully as it is a legally binding document. Initial all paragraphs and complete the last page. All fields are mandatory. I/We, the undersigned, agree to be bound by and to comply with the following terms and conditions in relation to above animal of which I am in receipt from Standard Poodles In Need, Rescue & Re-home, hereinafter referred to as SPIN.

Terms & Conditions*
- I/We will not use the dog for the purposes of breeding, experiments or vivisection. I/We will keep the dog as a household pet, and in my personal care.
- I/We will have the dog spayed/neutered when the dog attains the age of nine (9) months, unless the procedure has already been completed. I/We will keep this dog in good health and will provide veterinary care on a regular basis, including necessary vaccines.
- I/We understand SPIN, from whom I/we have obtained this dog, does not have a full-time veterinarian on staff, and that this animal has only received veterinary attention if required. Every precaution has been taken to ensure this dog is in good health, I/we understand that the dog I/we am adopting may be in need of additional medical attention. This may include, however is not limited to, respiratory infection, fleas, internal parasites, ear mites, etc. Any cost incurred will be my responsibility as the new owner. I/We agree SPIN will not be responsible for reimbursing any costs incurred by me for veterinary treatment.
- I/We understand that I/we am responsible for taking the animal I/we have adopted from SPIN to a veterinarian within 10 business days for a complete health examination. When examined by a veterinarian, if the animal is found to have a serious medical condition, I/we may return the dog to SPIN. A full refund for the adoption cost of this dog will be reimbursed only with documentation from a veterinarian of the medical condition within two weeks of adoption, or I/We will treat the dog at my expense.
- I/We will treat the dog humanely, and will not allow the dog to roam unattended. I/We agree that except for short periods of time, my dog will not be chained outdoors, nor will it be isolated in a confined area for long periods of time.
- If at ANY TIME during the dog’s life it must be relinquished, or I/we can no longer afford medical care, I/we agree to consult SPIN before turning the dog over to any other Humane Society, Municipal Pound or Person and to return the dog to SPIN upon their request.
- If I/we return the above dog to SPIN for any reason, I/we will at no time assert any claim, charge or demand of any kind or nature against SPIN for any charges that may have been incurred by me, including veterinarian fees, food, boarding, and training, in connection with said animal
- I/We will not use shock collars, choke/snake collars, prong/pinch collars, bark collars/citronella spray collars or other punitive training devices on the dog. Further, I/we will not use punitive training techniques (i.e. alpha roll, choking, etc.) on the dog.
- I/We will allow a representative of SPIN to examine the dog and the conditions in which the dog is kept and/or telephone from time to time after the adoption has taken place, and agree that if any of the above conditions have been violated, SPIN may repossess the dog. By signing below, I/we certify that I/we agree to the terms of this contract and that I/we am now fully responsible for the ownership of this animal being adopted from SPIN.
- I/We understand that animals are unpredictable and SPIN cannot anticipate or insure against unexpected conduct of animals received from SPIN. I/We understand and accept that any animal has the potential and ability to bite, and that SPIN cannot be held responsible for any injury to any person or animal arising from the Adopter’s decision to bring this animal into their home, however caused. I/We acknowledge that SPIN has not made through its agents, offers, volunteers or employees, any warranties regarding the future condition, temperament or conduct of the above animal. I/We hereby accept above animal as is, assume all risks and responsibilities associated with the custody and/or ownership of above animal, including bites, and I/we hereby fully and completely release, indemnify and hold harmless SPIN, its directors, offers, volunteers, agents, servants and employees from any claim, cause of action or liability of any sort of nature, whether known or unknown, directly arising out of or in connection with the adoption, care, ownership, maintenance, retention, temperament, conduct or condition of above animal.
